Last resort: declare any path template expression the assembled spec still leaves undeclared.

Runs over the finished spec rather than inside a parser precisely because it must not depend on which parser produced the operation — an OpenAPI file with a thin paths block, a framework we add next year, or a cross-file pass that rewrote a path and forgot its parameters all land here. Names are taken verbatim from the published path, because a parameter whose name differs from the token by so much as a sigil leaves the operation just as uncallable as it was.

Returns how many it had to invent, which is the number to watch: every one of them is a parser that should have known better.
