Reader-identity envelope (P0-b) — the ONE token/claims shape shared by A3 (access grants), C3 (reader accounts), and D4 (segmentation). See docs/architecture/reader-identity.md. A3 is the first implementer, so this file defines the contract the later features EXTEND (adding to the claims object) without changing the envelope.
